You can print it out and bring it in with you when you come in for your visit.

For new clients or existing clients with new pets: Please make sure you arrive 10-15 minutes prior to your appointment time. Please remember to bring any vaccine or medical record history with you. Also please remember to bring a fecal sample so we can check for internal parasites.
