International Health Certificate Information


If you plan on traveling abroad with your pet you can obtain your International Health Certificate at Oakhurst Veterinary Hospital.

This process typically requires one phone consult with the doctor and one office visit. Additional services such as vaccines or medications may also be required.

 

Obtaining USDA/Country Requirements

Before contacting OVH you need to obtain the following information

1). Visit www.aphis.usda.gov  (this is the agency in charge of Health Certification).

Using this site you can research the specific requirements of the country you are planning on visiting.

2). Contact the Consulate for the country that you are planning on visiting.

3). Contact the airline that you are flying on for specific airline requirements.

 

In addition we will need the following information from you when you call our office:

  1. Are you a NJ resident? If not you can not obtain a certificate from OVH.
  2. Country of destination
  3. Date of departure
  4. Name and telephone number of current veterinarian
  5. Best time that you can be contacted for your phone consultation with the doctor and telephone number that you would like us to call.
  6. USDA/Country Requirements* (see above)
  7. A deposit will be taken before a phone consultation / office visit is scheduled.
